How to See if You Qualify
Bring the following to the YMCA front desk (Mon–Fri, 8am–7pm):

Your Oregon DMV Photo ID with proof of physical address

Your Umpqua Health Alliance (UHA) card (if you have a copy)
Program Guidelines
Attendance Requirements

You must visit the YMCA at least 8 times per month to be considered for continued coverage.

Your membership will automatically renew at the end of the 3-month period if:

You’ve used it 8+ times per month, and
-
You are still a UHA CCO member
Excused Absences

If you're unable to meet the attendance requirement due to hardship:

Membership Policies

All YMCA policies and guidelines must be followed.

Membership may be terminated at any time if policy violations occur.

This program is a partnership between the YMCA and Umpqua Health Alliance under the Flexible Services Program.
