Are you a Umpqua Health Alliance (UHA) member? You may qualify for a special YMCA membership through our Flexible Services Program!

How to See if You Qualify

Bring the following to the YMCA front desk (Mon–Fri, 8am–7pm):

  • Your Oregon DMV Photo ID with proof of physical address

  • Your Umpqua Health Alliance (UHA) card (if you have a copy)

Program Guidelines

Attendance Requirements

  • You must visit the YMCA at least 8 times per month to be considered for continued coverage.

  • Your membership will automatically renew at the end of the 3-month period if:

  • You’ve used it 8+ times per month, and

  • You are still a UHA CCO member

Excused Absences

  • If you're unable to meet the attendance requirement due to hardship:

  • Email: agries@ymcaofdouglascounty.org

  • Call: 541-440-9622

  • Be sure to provide a reason for your absence.

Membership Policies

  • All YMCA policies and guidelines must be followed.

  • Membership may be terminated at any time if policy violations occur.

Additional Information

  • This program is a partnership between the YMCA and Umpqua Health Alliance under the Flexible Services Program.

  • "Flexible Services" are non-State Plan, health-related benefits intended to reduce costs and improve care and outcomes.
